TO ALL OWNER-OPERATORS – SUBCONTRACTORS

It is mandatory that all owner-operators and sub-contractors supply us with the following documents on an annual basis:

  1. W.C.B. Clearance Letters (we can obtain this letter if we have your W.C.B. # on file)
  2. Proof of Liability Insurance (minimum $2,000,000)
  3. Owner-operator or Sub-contractor Registration Form

It is also imperative that you provide us with any changes to your address or telephone numbers.
